Please click here to download your CTLE approval form. Before filling this form out, you should:

- Sign into MyLearningPlan (now Frontline PD) at mylearningplan.com

- Click on the My Portfolio link on the sidebar

- Select View Current Year from the sidebar

At this point, you can EITHER:

- Click File->Print at the top of the page and print the webpage

- Click the Export button at the bottom of the page, which will export your hours and courses to an Excel file, then print that spreadsheet

- Once you have your hours printed, please complete this form and attach it to the printout you generated from MyLearningPlan
